ANNOTATION

Over the past decades, the prevalence of dental anomalies and deformities has

increased dramatically, both among children and adults. Today, in 84% of

cases, orthodontic treatment is carried out using bracket systems. The

effectiveness of this technique makes orthodontists revise the usual age

indicators, tactics and methodology of treatment of various types of orthodontic

pathology. The presence of orthodontic equipment in the oral cavity can disrupt

the hygienic state, become a source of accumulation of dental deposits, and also

contribute to the development of inflammatory processes in the oral cavity. In

the process of orthodontic treatment in the oral cavity, the structure of the

microbiocenosis changes, which may contribute to an increased risk of

cryogenic and gingivopathogenic situations in the oral cavity. Everything

presented above determines the relevance and timeliness of this study.

Key words: bracket systems, orthodontic arches, shape memory effect (EZF),

instrumental-surgical method, inter-occlusal contact.

Restoration of individual aesthetic norms and functions of the

maxillofacial area is one of the fundamental tasks of orthodontics and prosthetic

dentistry. Over the past decades, the prevalence of dental anomalies and

deformities has increased dramatically, both among children and adults.

Various forms of occlusion abnormalities and secondary deformities associated

with violation of the integrity of the dentition of various etiopathogenesis are

among the most frequently diagnosed pathologies in clinical dentistry. Modern

studies have revealed a high degree of prevalence, variety of clinical

manifestations of occlusion abnormalities associated with violation of the

integrity of the dentition.

Modern views on the complex treatment of patients with various forms of

occlusion abnormalities are based on the postulates of the need to form a

morph-functional-aesthetic optimum, since the nature of the occlusive

relationship directly affects the quality of orthodontic and (or) orthopedic

treatment. Morph functional acceptable and stable in the time factor the results

of treatment of occlusion abnormalities and acquired deformations can be

obtained solely integrated (interdisciplinary) way. Only the apparatus-surgical

method of treatment of occlusion abnormalities can provide physiological

occlusion and restoration of full function regardless of the patient's age, which

is an absolute requirement for the completion of treatment, as only a

harmonious occlusion is a natural retention apparatus and prevents recurrence.

The use of various methods of treatment of occlusion anomalies and

secondary dent alveolar deformities and their high efficiency indicate that

orthodontic and complex treatment is widespread in orthodontics and (or)

orthopedic dentistry and has taken its rightful place.

Application, with the purpose of creating a temporary skeletal supports,

new medical technologies (MT) in daily clinical practice, such as removable

braces, orthodontic arc wire with memory effect of the shape (MES) units with

lingual attachment of locking elements, mini-implants (MI) have allowed to

expand the indications for the complex treatment of abnormalities of occlusion

and to improve the efficiency of rehabilitation of patients. To date, in 84% of

cases, orthodontic treatment is carried out using braces. The effectiveness of

this technique makes orthodontists revise the usual age indicators, tactics and

methodology of treatment of various types of orthodontic pathology. The

presence of orthodontic equipment in the oral cavity can disrupt the hygienic

condition, become a source of accumulation of dental deposits, and also

contribute to the development of inflammatory processes in the oral cavity.

In the available modern Russian literature, there is still no data on

algorithms, protocols and results (immediate and remote) of complex treatment

of patients with combined pathology with the use of modern fixed orthodontic

arc devices, orthodontic mini-implants and other devices, the terms of the

retention period used in this apparatus. The combination of these problems, of

course, makes scientific research in this area is very relevant, which determines

the relevance and timeliness of this study makes it possible to improve the

diagnosis, prediction and treatment of dental deformities in school-age children.

Available in a number of domestic and foreign literature data on the

prevalence of occlusion anomalies range from 11.4% to 71.7% .[6, 15, 19].

By the end of the formation of a permanent bite occlusion anomalies and

acquired dent alveolar deformities, as well as functional disorders caused by

them, acquire a pronounced stability, predetermined by a decrease in the

processes of restructuring in the bone tissue in the jaw bones [1,9].

As a result of the completion of the formation processes of the

maxillofacial area and reduce the reactivity of the bone, accelerated hardware

treatment of abnormalities of occlusion can cause the development of traumatic

occlusion, therefore, it is considered that, the later starts of orthodontic

treatment of anomalies of occlusion, so it is more complicated[2,13,21].

Apparatus-surgical treatment of occlusion abnormalities can provide

physiological occlusion and restoration of full function regardless of the

patient's age, since only harmonious occlusion prevents recurrence[1,3].

Disadvantages of traditional orthodontic devices led to intensive search for

new techniques and methods of treatment of occlusion anomalies. This method

was the use of modern fixed orthodontic arc devices - edgeways technique [10,

20].

The main principle of fixed orthodontic, edgeways - technique on the

example of technology of the direct arc (TDA) (Straight Wire-technic) is the

use of a programmable lock fixtures (Levell-arch, Triple-control or Straight- wirebracet) and straight orthodontic wire arc round, square, rectangular, not

requiring a transformation of the form of the wire [4, 18]. At the stage of

diagnosis and prognosis of treatment, it is very important to choose the area of

the jaw bone that is optimal for implant placement [12,16]. Objective

information can be obtained by analyzing the x-ray state of the bone tissue
